A synagogue builds a vibrant community by responding to the skills, needs and interests of its members. To help us respond to our members more effectively, we have created a Survey that will seek information about your expertise, interests and program preferences.
Please fill-out the survey in one of two ways:
- You can download the survey form (in .pdf format). Once you download it, you can print it, fill it out by hand, and mail it to CSI. To download the survey form, click PDF file.
- If the survey has been emailed to you, you can print it, fill it out by hand and mail it back to CSI.
